If a patient with acute chest pain, dizziness and shortness of breath is admitted to a hospital, doctors must quickly and reliably determine whether it is a heart attack. Due to lengthy examinations and complex laboratory diagnostics, however, patients usually have to be observed for many hours until a definite diagnosis can be made. A team of researchers from Basel has now successfully developed a mathematical formula that can be used to diagnose a heart attack within an hour.

If a heart attack is suspected, patients have to wait a long time for a diagnosis. If a heart attack is suspected, the electrocardiogram (EKG) often decides whether there is a blockage in the coronary arteries. Since the results are not always clear, doctors also carry out laboratory diagnostics. It is investigated whether troponin, a protein that is released by the dying heart muscle, can be detected. The result is often up to eight hours away. Those affected must therefore be observed for hours.

Tobias Reichlin from the University of Basel and his team have now successfully developed an algorithm that doctors can use to determine whether a patient has actually suffered a heart attack. The mathematical formula has been tested in 872 people with typical heart attack symptoms such as chest pain. In 77 percent of the cases, the suspected diagnosis was confirmed within the first hour. The calculation requires a serial blood sample, in which the blood values ​​of the heart protein troponin are measured.

The cells in the heart muscle begin to die 20 to 30 minutes after the blood flow has been interrupted. In the period of three to six hours, irreversible necrosis of the affected muscle tissue has developed. A heart attack can be triggered by physical exertion as well as stressful situations.
